**Added**

- Docs site with quickstart, concepts, dataset pages, MCP setup guides, example agents and a
  generated API reference. Every page has a Markdown copy (`.md`), plus `llms.txt` and
  `llms-full.txt`.
- MCP server (`@healthcare.dev/mcp`): 9 read-only tools, remote at `mcp.healthcare.dev/mcp` and
  local over stdio.
- TypeScript SDK (`@healthcare.dev/sdk`) and CLI (`@healthcare.dev/cli`, `hdev`).
- API v1: codes (ICD-10-CM, ICD-10-PCS, HCPCS) with search, hierarchy, batch and `as_of`; drugs
  (NDC, RxNorm, unified search); datasets, change feed and status.
- API keys, per-plan rate limits with `RateLimit-*` headers, and monthly quotas.
